Sips for Saints Restaurant Agreement Form

If you would like to participant in the Rotary Club of Alexandria Central Foundation’s Sips for Saints event, please complete the form below:

I agree to participate in the Sips for Saints at The Atrium, 277 South Washington Street, Alexandria, Virginia 22314 on March 13, 2020 and agrees to the following:
1. To supply 250 tastes of food specified below:
2. To provide these tastes from 7:00 until 9:30 pm;
3. To provide one (1) to three (3) restaurant representatives promptly from 6:00 pm until 9:30 pm to set up, serve and remove food;
4. Rotary Foundation Club of Alexandria Central Foundation will provide draped tables and identifying signs;
5. Restaurant will provide plates/bowls, napkins, and forks/spoons necessary for tastes served. Restaurant will provide serving dishes and serving utensils;
6. Restaurant understands that ice, electricity and/or heating/cooking equipment will not be provided and that propane is prohibited by the fire marshal;
7. Restaurant will provide electronic copy of restaurant logo to Rotary Club of Alexandria Central Foundation for event promotion and recognition.
8. To defend, indemnify, and hold the Rotary Club of Alexandria Central Foundation and its officers, employees, agents and volunteers harmless from any and all liabilities, losses, claims, fines and damages of any kind, including reasonable attorney’s fees, directly or indirectly, arising out of the restaurant’s participation in the provision of food during Sips for Saints. The purchase of insurance shall in no event be constructed as a discharge of liability;
9. Rotary Club of Alexandria Central Foundation shall assist with overall event-related VA health permit. Restaurant will provide menu and food handling information to Rotary Club of Alexandria Central Foundation representative for completion of health permit applications and will cooperate with health department as needed to obtain VA health permit for event.
